Backend Engineer (Java, FHIR, AWS, Healthcare) (Remote - Latam)

This position is posted by Jobgether on behalf of Darwoft. We are currently looking for a Backend Engineer (Java, FHIR, AWS, Healthcare) in Latin America.

We are seeking a talented Backend Engineer to join a healthcare-focused data platform team, building scalable, high-performing services for clinical and financial data integrations. In this role, you will work with FHIR-based workflows, distributed systems, and cloud architectures to enable secure and compliant data exchange. You will collaborate with cross-functional teams to design robust ingestion pipelines, optimize system performance, and ensure adherence to healthcare compliance standards. This position offers a hands-on opportunity to impact real-world healthcare solutions affecting millions of patients. Remote work across Latin America is supported, with alignment to the US-based team's schedules.

Accountabilities

• Design and implement scalable ingestion pipelines for FHIR-based clinical and administrative data
• Architect distributed backend systems using Java, Spark, Kafka, and Kubernetes
• Develop APIs and services to validate, transform, and enrich healthcare data
• Ensure privacy, security (HIPAA), and full data lineage throughout the integration stack
• Optimize batch and real-time data flows for latency, throughput, and cost efficiency
• Collaborate with product, DevOps, and architecture teams to deliver critical platform features
• Stay current with FHIR (R4, R5), US Core, and TEFCA standards, guiding internal adoption and best practices
  
  
  

Requirements

• 5+ years of professional software engineering experience
• 2+ years of experience with healthcare data integrations, particularly FHIR
• Expertise in Java, JVM optimization, and distributed frameworks such as Spark or Flink
• Hands-on experience with FHIR resources, bulk-data, SMART on FHIR, and HL7 v2/CDA
• Experience with Kafka, REST APIs, and event-driven architectures
• Familiarity with cloud services, preferably AWS (Lambda, EKS, S3, RDS, etc.)
• Understanding of HIPAA, data security, and healthcare compliance
• Strong problem-solving skills and ability to explain technical solutions to technical and non-technical stakeholders. Nice to have:
• Experience with backend languages like Python or Go
• Familiarity with FHIR Bulk API, Kafka Streams, or Kafka Connect
• Exposure to Terraform, GitHub Actions, Argo Workflows, or Helm
