Exciting Opportunity: 2 Physiotherapists - Angus Community Physiotherapy Service
We are delighted to offer an exciting new opportunity to work in Angus within a brand-new, innovative service. Angus Health and Social Care Partnership is redesigning the way we deliver care, focusing on a model that brings together multi-agency teams to prevent unnecessary hospital admissions and support people to remain at home.
Our ambition is to ensure that individuals who do not require acute hospital admission or who are already in hospital but no longer need acute care are redirected and supported to receive the Right Care, at the Right Time, in the Right Place.
We are seeking 2 experienced physiotherapists who are ready for a new challenge and eager to contribute to the reshaping of community services through the Frailty at Home and Home First Angus Single Point of Contact initiatives.
These are new posts and will sit within the Angus Community Physiotherapy team and offer a unique opportunity to work closely with multidisciplinary teams, including Occupational Therapists, Enablement response teams and Medicine for the Elderly advanced nurse practitioners (ANP’s).
Posts are 37 hours per week over 7 days
There will be a requirement for weekend working as part of a shift pattern. Travel throughout Angus is essential.
